The Reablement Care Service provides short-term rehabilitative support to people living in the Derry City Council area, recently discharged from hospital.

This service is made available 365 days a year and caters for approximately 20 service users at any given time.

Service users avail of the Reablement Care Service following e.g. a road traffic accident, after undergoing hip replacement surgery or any similar medical procedure. PCW’s dedicated team of staff assist Service Users to make a swift recovery to independent living.
